Class 16 - Requesting Information

Class Definition: How a person can make a request for information

To ensure that the Trust provides you with the right information as quickly as possible and to assist you in making your request, please consider the following:

Is the information you are about to request personal information about you or an individual of whom you are a carer/parent/guardian? If the answer is yes, this information is exempt under the Freedom of Information Act 2000 (It may however be obtainable under the Data Protection Act Subject Access )

Check to see if the information is made available through the Trust's Publication scheme, if you need information in hard copy please contact the Publication Scheme Coordinator

All requests for information under the Freedom of Information Act 2000 must be in a permanent form, ie. in writing or by email

All requests must include your name and address for correspondence. Please try and include as much information as you can, to help our Trust identify the specific information you require, Please supply:

  • Title (Mrs / Mrs / Miss etc)
  • First Name & Surname
  • Address & Postcode
  • Tel no/s - home / work / mobile
  • Email address
  • The Information Request Details
  • How you would like us to respond (email / phone / post)

Cost of Information

Some information is available free, but for others there may be a charge. We may charge you for hard copies or copying onto media (e.g. CD ROM). The charges will vary according to how information is made available.

What will happen when you make the request?

  • One, several or all of the items listed below could happen when a request is made for information:
  • We will acknowledge receipt of your request for information under the Freedom of Information Act 2000 within 3 working days
  • We may ask you to provide us with more information to help us locate the information you have requested
  • We will notify you if all or part of the information you have requested cannot be provided. If this happens we will explain to you why this information has been withheld.
  • We will notify you as soon as possible if we find we are unable to provide the information you have requested within 20 days
  • We will endeavour to provide you with the information you have requested within 20 working days
