The team provides nursing care to children within South Tyneside aged between 0-16 years or 19 years with special needs. The children may have acute or chronic health care needs which require a nursing assessment or intervention.
We accept referrals from all health care professionals, from local, regional and specialist hospitals.
Our main aim is to prevent or shorten hospital inpatient episodes and to promote family centred care. We do this by providing direct nursing care, education, support and liaison between health care and other professionals, to support the child whatever their surroundings.
The team comprises of specialist practitioners who are educated to degree level in community health studies and are nurse prescribers. We also have community staff nurses with a registered children nursing qualification and community nursery nurses.
