The Community Matrons are a team of nurses working in Sunderland and South Tyneside who Case Manage adults with more than one complex, long term condition (such as: diabetes, respiratory disease, heart disease).Community Matrons visits people in their own home or usual place of residence (e.g. Care Home). By working with people with complex needs, and their carer/relative they develop a personalised care plan and co-ordinate care with other health and social care professionals. By doing this they can manage their condition and prevent unnecessary admission or attendance at hospital, and if admission is required, they will work with the ward to support discharge home.
The Community Matron Service is available from Monday to Friday 8.30am to 5pm.
