Discharge Nursing Team

The Discharge Nursing Team was set up to support safe, timely discharge planning within South Tyneside District Hospital, in conjunction with the Hospital Discharge Policy.  

The Team consists of a Safe Care Lead and five Senior Discharge Planning Nurses with Admin support.  The focus of the team is to support ward staff with both simple and complex discharges, working in collaboration with other members of the multi-disciplinary team across primary and secondary care.

Discharge planning nurses work closely within Accident and Emergency department and Emergency Assessment Unit to prevent hospital admissions/ re-admissions, working closely with the Intermediate Care Team and hospital social work team to ensure appropriate care pathways.

Our role is to:

  • Provide specialist expertise in relation to discharge planning
  • Prevent admissions/ readmissions
  • Monitor/ Prevent delayed discharges
  • Ensure safe, timely discharge and reduction of length of stay
  • Provide training to other professionals in relation to discharge planning and continuing health care processes
  • Act as a specialist resource between primary and secondary care
  • Chair complex case conferences to ensure safe discharge planning
  • Attend daily ward based clusters and multi-disciplinary teams to support discharge planning process

The service is provided 7 days per week  - Monday-Friday 8-8pm, weekends/Bank holidays 8.30-4.30pm
